This survey has been developed to gather anonymous information that will enable us to gain a better insight into the Caribbean and African community in Greater Manchester

By completing this survey you are providing consent to the Caribbean and African Health Network to use this data to inform the aim of this actvitiy.

Your individual responses would not be used in any way that would allow your identification during any part of the report.

We are committed to protecting your personal information and respecting your privacy. Personal information is defined as any details that will enable you to be identified, such as reference numbers, telephone numbers, address, email address etc. The information will be safely stored on a protected database for up to 5 years.

In designing and executing our research findings, it is our policy to take all necessary steps to ensure that personal information you provide is safe and is not accessible to third parties.

The findings from the survey report will be disseminated for the purpose stated above.

Dear Participant

The aim of the scoping exercise is to gather essential information that will provide insight of where the Black community lives across the 10 localities of Greater Manchester.

The information gathered will give an overview of the community and enable  us better articulate the health and wellbeing needs of the Caribbean and African community. The information will be useful in engaging with service providers and commissioners across Greater Manchester. The findings from the survey will be published on CAHN's website and followed by a launch event where the community will determine next steps.

This survey adheres to ethical guidlines and all information gathered will be treated in the strictest confidence. Please do not provide your name, individual or personal identifiable data, we do not require this information to be collected as part of this survey.

There are some questions relating to your knowledge of organisations that largely address the needs of Caribbean and African people in Greater Manchester, we would appreciate the organisational details you are aware of so that we can identify what services exist and where they are located within Greater Manchester.

Thank you for the time you have given to complete this survey, a report will be published on our website when the exercise has been completed. The findings from this survey will inform conference, journal proceedings and relevant organisations seeking to understand community needs.
