This survey has been developed to gather information that will enable us to gain a better insight into the organisational services provided Caribbean and African led organisations in Greater Manchester.

By completing this survey you are providing consent to the Caribbean and African Health Network to use this data to inform the aim of this actvitiy.

We are committed to protecting your organisational information and respecting your organisations privacy. Organisational information is defined as any details that will enable you to be identified, such as reference numbers, telephone numbers, address, email address etc. The information will be safely stored on a protected database for up to 5 years.

In designing and executing our research findings, it is our policy to take all necessary steps to ensure that the organisational information you provide is safe and is not accessible to third parties.

The findings from the survey report will be disseminated for the purpose stated above.

Dear Participant

The aim of the scoping exercise is to gather essential information that will provide an overview of available services predominantly serving the needs of the Caribbean and African community across the 10 localities of Greater Manchester. The information gathered will enable us to better articulate the needs of our community as we engage with service providers and commissioners.

This survey adheres to ethical guidlines and all information gathered will be treated in the strictest confidence. The information collected as part of this survey will be used for the purpose stated above.

The findings from this survey will inform conference, journal proceedings and relevant organisations seeking to understand community needs. A report will be published on our website when the exercise has been completed. No personal or named organisational information will be included in public reports, published material or discussions with other agencies.
